What next flight out means
Next flight out (NFO) is a premium expedited air service in which a shipment is booked onto the next commercially available departure serving the origin and destination airports, rather than moving through a carrier's own scheduled overnight network. A ground courier drives it to the airport, it is tendered as air cargo, it flies, and a second courier recovers it at the far end and drives it to the consignee.
The distinction from overnight express matters. Overnight moves a parcel through a fixed hub-and-spoke network on a published schedule. NFO ignores that network and buys space on whatever leaves soonest. One is scheduled and deferred, the other reactive and opportunistic, which is why NFO is quoted per lane and per departure rather than as a standing service level.
Where the time actually goes
Most providers sell the flight. Three of the four legs happen on the ground, and they are the ones that decide the outcome.
Ground pickup to the origin airport
A courier collects from the site and drives to the airline cargo facility. This leg is where the clock is usually lost or saved, because it has to beat the airline's cargo acceptance cutoff for the flight you are aiming at.
Tender and acceptance
The shipment is tendered as air cargo and screened. This is the leg that decides whether the shipment flies at all, and it is the one a laboratory cannot perform for itself unless it has been vetted.
The flight
The consignment travels in the belly of a passenger aircraft or on an all-cargo flight. This is the part nobody controls, which is why the honest measure of an NFO provider is what happens either side of it.
Recovery and final delivery
A second courier recovers the shipment at destination and drives it to the laboratory, with the delivery reconciled against the original dispatch record.
What actually stops a specimen flying
Six things decide whether a shipment departs on the flight you planned. None of them is speed.
A lab usually cannot hand its own box to an airline
Under 49 CFR 1544.205, an aircraft operator may accept cargo only from the shipper, another aircraft operator or foreign air carrier, an indirect air carrier operating under an approved TSA security program, or a certified cargo screening facility. DS2 holds TSA Indirect Air Carrier status under 49 CFR Part 1548, so it can tender your specimens into commercial air cargo on your behalf rather than requiring every hospital or lab to become its own vetted shipper.
Screening can mean opening the box
Air cargo screening includes physical examination. A packout designed only for a smooth journey can be defeated by a legitimate inspection, so cold-chain configuration and the paperwork that travels with it have to survive being handled, not just being flown.
The most common reason a cold shipment is refused
Dry ice is a Class 9 dangerous good, UN1845, and it is declared, weighed and limited by the operator. Getting the quantity, venting and documentation right at the counter is routine work for a trained shipper and a hard stop for anyone improvising.
Quantity rules decide whether it can fly
Category B specimens ship under 49 CFR 173.199 in triple packaging, and for air the limits are 1 L per primary receptacle and 4 L per outer packaging, with the UN3373 mark required to appear entirely on one side of the package. Those numbers are not negotiable at the counter.
Some specimens are not dangerous goods at all
A useful and frequently missed point: many routine specimens fall outside the Division 6.2 rules entirely under the exceptions at 49 CFR 173.134(b), which changes the packaging, the cost and sometimes the speed. Classifying correctly is cheaper than over-packing by default.
The air waybill is not your accession number
An NFO shipment generates an air waybill that has no relationship to the laboratory's specimen identifier. Somebody has to tie the two together, or the specimen arrives with a tracking history the lab cannot match to its own record.
A note on what IAC status is and is not: an indirect air carrier is by definition not an airline and holds no FAA air carrier operating certificate. It means DS2 operates a TSA-approved cargo security program and can tender cargo into the air system on a client's behalf. The airline still owns the aircraft, the schedule and the final acceptance decision.
Speed is a clinical variable, not a service badge
Time-critical analytes degrade on a clock. In a 201-donor study published in PLoS ONE in 2024, potassium in unseparated lithium heparin plasma rose a mean of 0.62 mmol/L by four hours and 1.21 mmol/L by eight, with 164 of 201 donors meeting criteria for pseudohyperkalemia at eight hours. Whole blood lactate rises roughly 0.008 mmol/L per minute at room temperature against 0.001 on slushed ice. Transport time and transport temperature are pre-analytical variables, and they change the result.
Turnaround also moves patients. A 2022 study of 23,718 emergency department visits found that each additional minute of laboratory turnaround time was associated with 0.32 additional minutes of emergency department length of stay, and that the analytical stage accounted for a median of only 69% of total turnaround. The remaining third sits in collection, handling and transport. That study measured turnaround inside a single hospital rather than courier transport between facilities, and it shows association rather than causation, but the direction is clear enough: the part a logistics program controls is not a rounding error.
When NFO is the wrong answer
It is an expensive service, and it is not always the fastest one. Three cases where something else wins.
A vehicle can make it in time
If a dedicated ground run fits inside the analyte's stability window, it is faster door to door, cheaper, and keeps continuous custody with no airline handoffs.
No suitable flight exists
NFO is only as fast as the next departure that serves both airports. On a thin lane late in the day, an overnight express service with a guaranteed morning commitment can beat waiting for a 6am flight.
The specimen cannot survive the packout
If the transit time plus airport dwell exceeds what the packout can hold, the answer is a different collection plan or a different laboratory, not a faster flight.

Can a courier carry a specimen in the passenger cabin instead?
Not for regulated specimens. A hand-carried consignment is still legally cargo: 49 CFR 1540.5 defines cargo to include all accompanied commercial courier consignments, whether or not accounted for on an air waybill. The passenger provisions at 49 CFR 175.10 contain no allowance for UN3373 Category B substances, and the passenger dry ice allowance is capped at 2.5 kg and applies only to perishables not subject to the hazardous materials regulations.
What decides whether NFO takes four hours or fourteen?
Three things, none of them the flight itself: whether the ground leg beats the airline's cargo acceptance cutoff, whether a suitable departure exists on that lane at that hour, and whether the shipment clears screening without being held. Flight time is usually the smallest and most predictable part of the total.
When is NFO the wrong choice?
When a dedicated ground vehicle can make it inside the stability window, because that is faster door to door and keeps continuous custody. When no suitable flight exists on the lane, because a guaranteed overnight commitment may beat waiting for the first morning departure. And when the packout cannot hold for the transit plus airport dwell, in which case the answer is a different plan rather than a faster flight.
