At Diagnostic Support Services (DS2), protecting the confidentiality and security of your Protected Health Information (“PHI”) is important to us. In accordance with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), we are legally required to maintain the privacy of your PHI and provide you with this Notice explaining our legal responsibilities and privacy practices.
PHI includes information we create or receive related to your medical condition, health care services, testing, or payment for those services. DS2 is committed to safeguarding your information and implementing reasonable measures to protect it as required under applicable law.

DS2 is required to:

DS2 reserves the right to revise this Notice and update our privacy practices at any time, as permitted by law. Updated versions will apply to all PHI maintained by us and will be posted on our website.
Complaints and Questions
If you believe your privacy rights have been violated, you may file a complaint with DS2 or with the U.S. Department of Health and Human Services Office for Civil Rights. Filing a complaint will not affect your care or result in retaliation.
